What this means for patients

The lateral ligaments support the outside of the ankle. Sprains can injure these structures, and recurrent injury may lead to giving-way or persistent instability. Evaluation includes ligament stability, muscle function, foot alignment and possible associated tendon or cartilage injury. Rehabilitation and bracing are often part of care; persistent symptomatic instability may lead to discussion of ligament repair or reconstruction.

When a sprain becomes recurrent instability

Repeated lateral sprains can leave the supporting ligaments loose or damaged. The ankle may give way on uneven ground or during sport. Continuing pain can also reflect cartilage or tendon injury, so recurrent symptoms deserve a broader assessment.

Build stability through rehabilitation

Treatment commonly includes rehabilitation of strength, balance and movement control, with a brace when appropriate. Examination distinguishes mechanical ligament laxity from other contributors such as muscle control or foot alignment. A high ankle sprain involves different structures and needs its own assessment.

Surgery and return to sport

Persistent symptomatic instability despite appropriate nonsurgical care can lead to ligament repair or reconstruction. Associated alignment or cartilage problems may influence the operation. Return to sport is guided by recovered function, stability and rehabilitation progress rather than a guaranteed calendar date.

Questions for your consultation

  • Is the instability related to ligaments, muscle control or alignment?
  • What rehabilitation should I complete before considering surgery?
  • What would a safe return to sport involve?
