What this means for patients

Toe deformities can create pressure against shoes, painful calluses and difficulty walking. Evaluation identifies which joints are involved, whether the deformity is flexible or rigid and whether there is an associated bunion or muscle imbalance. Footwear adjustment and pressure relief may help; correction is considered when symptoms and the structural problem warrant it. Dr. Noman A. Siddiqui’s research includes comparison of minimally invasive and open hammertoe correction.

Identify which joints are bent

Hammertoe affects the middle joint of a lesser toe; mallet toe affects the joint nearer the tip, and claw toe combines several abnormal joint positions. Toe crowding, arthritis or muscle imbalance may contribute. Assessment also establishes whether the toe is flexible or rigid.

Pressure relief is a useful starting point

A wider, deeper toe box and appropriate padding can reduce rubbing and painful calluses. Selected exercises or therapy may help with a flexible problem. A bunion or underlying nerve disorder may need consideration as part of the overall foot assessment.

When correction is considered

Persistent pain or walking difficulty despite nonsurgical measures can prompt discussion of correction. Tendon procedures, bone correction or joint fusion may suit different deformities. The choice between open and minimally invasive approaches depends on the structural problem; ask about fixation, swelling and footwear during recovery.

Questions for your consultation

  • Is the deformity flexible or rigid?
  • Can footwear or pressure relief improve my symptoms?
  • Would minimally invasive or open correction be appropriate?
