What this means for patients

Flatfoot describes a lowered arch. Some flat feet are flexible and painless; others develop pain, progressive deformity or arthritis in the foot’s joints. Assessment considers the arch, heel alignment, joint motion and tendon function. Depending on the findings, care can include footwear support, bracing and rehabilitation, or reconstruction. Arthritis and a stiff deformity can change the available surgical options.

A low arch is not always a painful problem

Some people have a longstanding flexible flatfoot without symptoms. Others develop fatigue, arch pain, difficulty standing or a progressive change in shape. The cause may involve tendon dysfunction, arthritis or an underlying developmental or neurological condition.

Assessing the whole foot

Examination considers the heel, arch, tendon strength and whether the deformity remains flexible. Standing imaging helps identify alignment and arthritic joints. Pain along the inner ankle may reflect posterior tibial tendon dysfunction, but the cause should be established rather than inferred from arch height alone.

Treatment follows symptoms and flexibility

Supportive shoes, inserts, bracing and rehabilitation may ease symptoms. Persistent pain or progressive deformity can lead to a reconstruction discussion. Flexible feet and stiff arthritic feet often require different procedures; the aim is a more comfortable, functional foot rather than a particular arch appearance.
